My earth lead is getting really hot when welding MIG spray arc at 27volts and around 210amps. Do I need to increase the size of my earth?

What size wire are you running for the lead and what amperage is the clamp rated for? Even if you are within the range, the earth will still get rather hot welding at those voltages.

Well, while you're dealing with that...my truck won't start. Should I change the battery?

could be undersized lead but also just poor connections. even to the table/part itself.
with mig you must have a good earth lead.
tweak it until it breaks

How hot? If it's hot enough to cause a fire then yeah I would definitely upsize your ground. if it's only like 150 deg F I wouldn't worry too much about it.
